The Silent Spine: Why Your Back Can Be in Trouble Before It Hurts

A systematic review published in the American Journal of Neuroradiology, examining 33 studies and 3,110 asymptomatic individuals, found that disk degeneration was present in 37% of pain-free 20-year-olds. By age 50, that number climbed to 80%. By age 80, it reached 96%. Disk bulges were found in 30% of pain-free 20-year-olds and 84% of pain-free 80-year-olds. Disk protrusions were present in 29% of asymptomatic 20-year-olds.
The authors concluded that many imaging-based degenerative features are likely part of normal aging and are unassociated with pain. In other words, a significant percentage of people are walking around with disc degeneration, disc bulges, and other structural changes on their spine imaging and feeling nothing.
This has two important implications. First, a finding on an MRI or X-ray is not the same as a diagnosis of the cause of your pain. Structural findings need to be interpreted in the context of the whole person, not read in isolation. Second, and more relevant to this discussion, the absence of pain does not mean the absence of a problem. The spine can be undergoing significant change long before the nervous system registers that change as pain.
